If you have salvage vehicle you must provide:

Physical Inspection from the State Troopers (located at office of Motor Vehicles 100 Veterans Blvd)

All receipts for every part replaced on the vehicle. (If parts came from another vehicle, you must have the VIN for the vehicle from which the parts are coming from

A completed salvage disclosure form
